Analysis on the prevalence and risk factors of skin injuries in the elderly in 50 hospitals in China

Objective To analyze the prevalence and risk factors of 3 types of skin injuries in the elderly through multicenter cross-sectional survey,so as to provide bases for formulating prevention strategies for skin injuries in the elderly. Methods On March 31,2021,1 067 nurses from 50 hospitals in 16 provinces,2 autonomous regions and 2 municipalities included inpatients over 60 years old and inspected their skin from head to toes to identify pressure injury(PI),incontinence-associated dermatitis(IAD) and skin tear(ST),and meanwhile collected demographic,healthcare condition and PI risk assessment data. Statistical software was used for descriptive analysis,Pearson correlation analysis and logistic regression analysis. Results Totally,14 470 cases were obtained. The average age was(73.35±8.90) years old. The total prevalence of skin injuries was 6.69%,and the prevalence of PI,IAD and ST was 3.91%,1.89% and 0.89%,respectively. Logistic regression analysis showed that the risk of PI,inability of self-care,hypoproteinemia and anemia,comorbidity over 2 chronic diseases and ≥70 years old were the independent risk factors of PI in the elderly. The risk of PI,inability of self-care,hypoproteinemia,comorbidity over 2 chronic diseases,length of hospital stay≥8 days and the age of over 80 years were the independent risk factors of IAD in the elderly. The risk of PI and hypoproteinemia were independent risk factors of ST in the elderly. Conclusion Skin injuries of the hospitalized elderly are the result of multiple factors,while PI and IAD are the most typical ones,and the risk factors of different types of skin injuries are also different. When formulating prevention strategies,we should not only pay attention to the independent role of risk factors,but also consider the combined role of risk factors,take comprehensive preventive measures to improve the prevention effect.

Effect of mindfulness-based stress reduction combined with exercise training on sleep in elderly patients with stable chronic obstructive pulmonary disease

Objective To investigate the effects of Mindfulness-based Stress Reduction(MBSR) combined with exercise training on sleep structure and sleep quality in elderly patients with stable chronic obstructive pulmonary disease(COPD),and to provide an objective reference for improving the sleep quality of patients with stable COPD in old age. Methods Using the convenient sampling method,23 elderly stable COPD patients who were admitted or re-diagnosed at a tertiary A hospital in Beijing from April 2019 to July 2020 were selected as the study subjects. Through self-control,patients were given a 6-week MBSR combined with exercise training interventions. Bracelet-type sleep monitoring devices,watch-based blood oxygen meters,and the Pittsburgh Sleep Quality Index(PSQI) were used to assess differences in sleep structure,nighttime oxygen saturation parameters,and sleep quality before and after the intervention. Results After intervention,the proportion of NREM 3 stage in patients increased from(9.26±2.39)% to(13.08±3.33)%;the lowest oxygen saturation at night increased from(84.05±5.40)% to (87.45±4.17)%;the Oxygen Desaturation Index decreased from(16.14±10.60) cycles/h to(9.95±6.12) cycles/h;the low oxygen ratio increased from[4.70(0.67,11.60)]% to[1.25(0.15,4.02)]%;the PSQI score reduced from(6.65±2.85) scores to(4.55±2.11) scores;the differences were statistically significant(P<0.05). Conclusion MBSR combined with exercise training can optimize the sleep structure and improve night oxygenation and sleep quality in patients with elderly stable COPD.

This study aims to summarize the nursing experience of 4 elderly patients with endotracheal intubation using automatic airway management system. The main points of nursing experience include the care of elderly patients with endotracheal intubation,the application of automatic airway management system,the care of intubation catheter cuff and subglottic secretion suction. No ventilator associated pneumonia occurred in the 4 patients during catheterization and they were successfully extubated.

Expert consensus on clinical application of disposable absorbent care products for adults with incontinence

Objective To form an expert consensus on the clinical application of disposable absorbent care products for adult incontinent patients. Methods Through systematically reviewing domestic and foreign literatures as well as combining with laboratory results,domestic experts were invited for evaluation,expert meeting method and Delphi expert consultation method were used to form the consensus combined with laboratory data and expert opinions,and all items were modified and improved. Results A total of 20 items were agreed on the index requirements and clinical application on disposable absorbent care products for adults with incontinence. Conclusion The established consensus can provide guidance on assessment,utilization,and evaluation of disposable absorbent care products for adult incontinent patients during hospitalization,and can provide clinical standardized guide for skin environmental protection for hospitalized incontinent patients.
